Zusammenfassung
Die Auswirkungen einer Beatmung auf das Gehirn sind weit weniger definierbar und damit auch weniger bekannt als die Beeinflussung anderer Organe. Oftmals ist in der Literatur vor allem bei vorgeschädigtem Gehirn ein diffuses, nicht exakt wissenschaftlich belegbares Unbehagen zu verspüren, insbesondere, wenn die Frage der PEEP-Beatmung angesprochen wird. Dies hatte zur Folge, daß bis in die 50er Jahre neurochirurgische Operationen vielerorts in Lokalanästhesie durchgeführt wurden und daß noch 1978 Richard und Karimi [26] ausführten: „Soweit es die pulmonalen Aspekte zulassen, beatmen wir in der Regel mit einem negativen endexspiratorischen Druck“. Dem stehen neuere Erkenntnisse entgegen, wonach bei gravierender zerebraler Schädigung eine Beatmung geradezu unabdingbar notwendig ist, um durch Hypoxie und Hyperkapnie ausgelöste sekundäre ischämische Schädigungen zu vermeiden. Im folgenden sollen die zerebralen pathophysiologischen Reaktionsabläufe als Ausdruck der Änderung von Atemparametern betrachtet werden sowie näher auf die Auswirkungen von IPPV und PEEP eingegangen werden. Im letzten Abschnitt wird zu Möglichkeiten der therapeutischen Beeinflussung eventueller negativer zerebraler Auswirkungen einer Beatmung Stellung genommen sowie das dafür notwendige Monitoring aufgezeigt.
