Zusammenfassung
Rückenschmerzen haben eine hohe Prävalenz und sind ein bedeutsames gesundheitsökonomisches Problem in Deutschland. Die Behandlung der Patienten erfolgt bisher allerdings weitgehend unstrukturiert: Bei vielen Patienten wird eine unnötige und teure Bildgebung durchgeführt oder ohne Indikation operiert, obgleich die Nationalen Versorgungsleitlinien Kreuzschmerz (NVL) diese Praxis explizit missbilligen. Das vom Innovationsfonds des Gemeinsamen Bundesausschusses (G-BA) geförderte Projekt Rise-uP möchte hier ansetzen und die NVL über E-Health-Technologie und eine mobile Anwendung (Kaia Rückenapp) realisieren. Die Rückenschmerzbehandlung erfolgt gemäß Rise-uP hausarztzentriert. Über eine gemeinsame elektronische Fallakte haben Schmerzspezialisten Zugriff auf die Patientendaten und können den Hausarzt bei Bedarf telemedizinisch beraten. Der Patient wird über die Kaia Rückenapp u. a. zu Eigenaktivität und Entspannungsübungen angeregt. Das Konzept wird in der Evaluation der Regelversorgung gegenübergestellt. Das Buchkapitel stellt das Rise-uP-Konzept ausführlich vor und diskutiert beispielhaft die Herausforderungen bei der Einführung von E-Health im Gesundheitssystem mit Fokus auf die Akzeptanz der Ärzte.
