Die Preisgestaltung von Arzneimittel aus internationaler Perspektive

Zusammenfassung

Der Gesundheitssektor ist ein stark staatlich regulierter Markt. Insbesondere die Ausgaben und die Preisgestaltung von Arzneimitteln sind Gegenstand anhaltender Diskussionen in nationalen und internationalen Kontext. In diesem Beitrag sollen die verschiedenen Bestandteile des Verkaufspreises und der Erstattungshöhe von Arzneimitteln einschließlich der länderspezifischen Unterschiede beschrieben werden. Patentgeschützte, innovative Arzneimittel unterliegen einer anderen Preiskalkulation und anderen staatlichen Einflussfaktoren als Generika ohne Patentschutz. Es wird ein Überblick über die verschiedenen international eingesetzten Instrumente der Preisgestaltung gegeben. Insbesondere wird auf die Ansätze des Value-based Pricing, der gesundheitsökonomischen Evaluation und der internationalen Preisreferenzierung bei neu in den Markt kommenden Arzneimitteln sowie Rabattverträgen und Festbetragsregelungen für Generika eingegangen und es werden die verschiedenen Ausgestaltungsmöglichkeiten von Managed Entry Agreements angesprochen. Abschließend zeigt der Beitrag die bestehenden und zukünftigen Herausforderungen der internationalen Preisgestaltung aus Public-Health-Sicht auf.
