Zusammenfassung
Angesichts steigender Zahlen von älteren, multimorbiden Personen mit chronischen Erkrankungen ist es geboten, die ambulante Versorgung für diese Personengruppe auszubauen. Am Beispiel der kardiovaskulären Erkrankungen wird exemplarisch aufgezeigt, dass die Kombination von Pflegewissenschaft und Gesundheitspsychologie ideale Voraussetzungen bietet, um hier neue Konzepte der pflegerischen Versorgung umzusetzen. Entsprechend des modernen, erweiterten Pflegebegriffs können spezialisierte Pflegekräfte etwa Patientinnen und Patienten mit chronischer Herzinsuffizienz auch nach stationären Klinikaufenthalten darin unterstützen, ihr Selbstpflegeverhalten oder Selbst-Management der Erkrankung zu verbessern. Dies umfasst gemäß aktueller europäischer Leitlinien zur Versorgung dieser Patientengruppe unter anderem korrekte Medikamenteneinnahme, Steigerung der körperlichen Aktivität, aber auch das Erkennen und Bewältigen psychischer Probleme. Ziel ist es, über diese verhaltensbezogenen Maßnahmen den Gesundheitszustand zu stabilisieren und Krankenhausaufenthalte zu reduzieren. Die Gesundheitspsychologie kann mit ihren theoretischen Modellen, Interventionsstrategien und -techniken dazu beitragen, die gewünschten Verhaltensänderungen, die hierfür nötig werden, dauerhaft in den Alltag zu integrieren. Es liegt bereits empirische Evidenz vor, dass Pflegekräfte derartige Maßnahmen auf der Basis gesundheitspsychologischer Ansätze erfolgreich umsetzen können. Pflegewissenschaft plus Gesundheitspsychologie liefert somit ideale Voraussetzungen, um eine moderne, leitliniengerechte und ganzheitliche Versorgung von Personen mit chronischen Erkrankungen settingübergreifend umzusetzen. Im Bachelorstudiengang Pflegewissenschaft - Klinische Pflege an der Universität Trier arbeiten die Abteilungen für Pflegewissenschaft und für Gesundheitspsychologie bereits erfolgreich zusammen. In einem konsekutiven Masterstudiengang soll dies zukünftig fortgesetzt und ausgebaut werden.
