Zusammenfassung
In den letzten Jahren ist allgemein die Vorstellung akzeptiert worden, daß die klinischen Symptome bei akutem Kreislaufversagen bedingt sind durch die sogenannten „Störungen der Mikrozirkulation”. Um diesen Begriff einzuengen, möchte ich ihn definieren als eine Verringerung des Erythrocytenfluxes pro Zeit durch nutritive Capillaren. Funktionsstörung im periphersten Glied des Kreislaufsystems kann auch dann noch die eigentliche Aufgabe, den Stoffaustausch, unterbinden, wenn andere, mehr zentral gelegene Kreislaufabschnitte in ihrer Funktion nur geringgradig oder gar nicht eingeschränkt sind. Bei aller Bedeutung der wichtigen Funktion von Herz und der großen Gefäße besagt ihre Funktion doch häufig wenig über den Funktionszustand der Capillaren. Auf der anderen Seite ist die Größe des diesem Kreislaufsystem zur Verfügung stehenden Blutvolumens sowie der arterielle Blutdruck für die Gesamtfunktion zwar kritisch; der Erfolg einer therapeutischen Volumenzufuhr bzw. Druckerhöhung wird allein daran zu messen sein, inwieweit durch sie die Normalisierung der Capillarperfusion gelingt. Es ist nur zu bekannt, daß dies keineswegs immer gelingt.
