Zusammenfassung
Die seltene Fruchtwasserembolie ist trotz der großen Fortschritte in der Medizin die lebensbedrohlichste geburtshilfliche Komplikation, die einer jungen Mutter widerfahren kann. Ihre Häufigkeit wird mit 1: 6000–1: 80000, ihre Mortalität mit 25–86% angegeben. Sie ist durch eine kardiorespiratorische Insuffzienz und anschließende Gerinnungsstörung charakterisiert. Die Patientinnen zeigen zunächst Dyspnoe, Agitiertheit und Angst gefolgt von Zyanose und Blutdruckabfall. Es folgt eine hypoxiebedingte neurologische Symptomatik bis hin zum Koma und Herzstillstand.
Der Nachweis von Fruchtwasserbestandteilen im Blut oder in der Lunge sichert die Diagnose nicht. Sofortiger Verdacht und Behandlung der Symptome durch ausreichende Oxygenierung, Volumensubstitution und Korrektur der Gerinnungsprobleme sind die wesentlichsten Merkmale einer Therapie. Ähnlichkeiten der FWE mit einem anaphylaktischen Geschehen lassen die Gabe von Kortikosteroiden sinnvoll erscheinen.
