Zusammenfassung
Der Schwangerschaftsabruch oder induzierte Abort wurde noch vor 25 Jahren in der allgemeinen Terminologie als krimineller Abort bezeichnet. Seitdem sind die Abortgesetze in vielen Ländern liberalisiert worden. Daraufhin sank die mütterliche Mortalität, und die Zahl der legalen Aborte stieg an. Im Gegensatz zu Japan, China und der damaligen Sowjetunion wurde der induzierte Abort in den westlichen Ländern immer als eine Notlösung und nicht als eine Methode der Familienplanung angesehen.
Die in den verschiedenen Gesetzen unterschiedlich geregelte zeitliche Begrenzung des Schwangerschaftabbruchs basiert auf der Feststellung, daß die Schwangerschaft mit der Implantation beginnt. Nach der 22. Woche post conceptionem (p.c.) ergeben sich besondere ethische Probleme, die auch die Abortbefürworter in 2 Lager spalten, da zu diesem Zeitpunkt der Fetus im allgemeinen lebensfähig geworden ist, d. h., er kann außerhalb des Uterus überleben.
In vielen Gesetzen, darunter auch dem neuen deutschen, ist keine embryopathische Abortindikation vorgesehen, die in der alten Fassung bis zur 24. SSW post menstruationem (p.m.) reichte. Die Notlagenindikation wurde bis zur Geburt ausgedehnt. War früher eine Notlage mit einer Gefahr für das mütterliche Leben verbunden, so ist jetzt eine soziale Indikation ausreichend. Diesen Umstand halten auch jene Geburtshelfer nicht für richtig, die die Indikation für den Frühabort grundsätzlich befürworten.
Soll die Implantation verhindert werden, wird eine Notfallkontrazeption durchgeführt oder ein Frühabort induziert.
Die meisten Abbrüche werden in der frühen Schwangerschaft bis zur 8. Schwangerschaftswoche (SSW) vorgenommen. Für diesen Zeitraum steht eine neue Methode des Abbruchs, der medikamentöse Abort, zur Verfügung. Als Medikamente haben sich Antigestagene (Mifepriston), Methotrexat und/oder Prostaglandine als wirksam erwiesen.
Bei älteren Schwangerschaften bis etwa zur 14./16. SSW wird das Schwangerschaftsprodukt nach Dilatation des Zervikalkanals mittels Vakuum abgesaugt. Prostaglandine, hygroskopische Stifte, aber auch das Antigestagen Mifepriston können die Zervix erweichen.
Nach der 16. SSW kann eine Abortausräumung durchgeführt werden, die in den USA seit kurzem heftig umstritten ist. Der Verzicht auf diese Methode fällt leicht, da für ihren Einsatz infolge moderner Prostaglandine keine Notwendigkeit mehr besteht. Durch Prostaglandine wird die Zervix erweicht, der Zervikalkanal erweitert, und es werden Wehen erzeugt, die zur Ausstoßung des Schwangerschaftprodukts führen, nachdem der Fetus abgestorben ist.
Mit zunehmender Schwangerschaftsdauer steigen bei allen Methoden Morbidität und Mortalität an (Binkin 1986).
