Vorzeitiger Blasensprung am Termin

Zusammenfassung

Etwa 8–10 % aller Schwangerschaften werden durch einen vorzeitigen Blasensprung am Termin kompliziert. Für die Diagnose eines vorzeitigen Blasensprungs haben sich neben gängigen Untersuchungsverfahren wie dem Nitrazintest biochemische Tests etabliert, die das fetale Fibronektin oder das IGF-bindende Protein im Fruchtwasser nachweisen.

Bei Vorliegen eines reifen Zervixbefundes, der mittels steriler Spekulumeinstellung erhoben werden soll, stellt die Geburtseinleitung nach einem Intervall von etwa 6 h die Therapie der Wahl dar. Einzig bei Nullipara und sehr unreifem Zervixbefund ist ein Zuwarten bei engmaschiger Kontrolle der Entzündungsparameter und Vermeidung digitaler vaginaler Untersuchungen möglicherweise von Vorteil, da die Anzahl operativer Entbindungen reduziert werden kann. Bei einem Intervall von mehr als 12 h zwischen Blasensprung und Geburt sollte eine Antibiotikaprophylaxe durchgeführt werden.
