Zusammenfassung
Jede Geburtseinleitung bedarf einer Indikation, die besonders bei unreifer Zervix streng zu stellen ist. Oxytozin wird als kontrollierte intravenöse Infusion bei reifer Zervix, vor allem bei klarer medizinischer Indikation angewendet; Prostaglandine können endozervikal und intravaginal als Gel oder Tabletten in unterschiedlicher Dosierung sowohl bei reifer als auch unreifer Zervix angewendet werden. Durch die lokale Applikation kommt es zu keinen nennenswerten systemischen Nebenwirkungen. Die besondere Wirkung vor allem von PGE2 bei der unreifen Zervix erklärt, daß die Anwendung dieser Substanz zur Geburtseinleitung in dieser klinischen Situation gegenüber Oxytozin deutliche Vorteile mit sich bringt.
Um Überstimulationen zu vermeiden, dürfen diese beiden Substanzen niemals gleichzeitig, sondern nur hintereinander mit einem Sicherheitsabstand von 6 h verabreicht werden - der gleiche Sicherheitsabstand, der zwischen mehrmaligen Prostaglandinapplikationen einzuhalten ist. Weder ein spontaner Blasensprung noch ein Zustand nach Sectio sind prinzipielle Kontraindikationen; bei letzteren ist aber die Indikation zur Einleitung besonders streng zu stellen und in jedem Fall eine Kombination von Prostaglandinen mit Oxytozin zu vermeiden.
Jede Geburtseinleitung bedarf einer kardiotokographischen Überwachung, deren Intensität vom klinischen Einzelfall abhängig ist.
Eine Hyperstimulation, die bei den verschiedenen Einleitungsverfahren unterschiedlich häufig auftritt, kann immer durch die intravenöse Gabe eines β-Sympatikomimetikums patikomimetikums zumindest kurzfristig durchbroehen werden. Andere Nebenwirkungen der Geburtseinleitung sind insbesondere bei der lokalen Anwendung von Prostaglandinen selten.
In letzter Zeit sind Berichte über das synthetische PGE1-Analogon Misoprostol erschienen, das vor allem wegen seiner geringen Kosten eine attraktive Substanz darstellen könnte.
