Zusammenfassung
Trophoblasterkrankungen umfassen die komplette und partielle hydatiforme sowie die destruierende invasive Mole, den plazentanahen Pseudotumor und das Chorionkarzinom. Die hydatiforme Mole findet sich in 1 von 1000, das Chorionkarkinom in 2,46 von 100000 Schwangerschatten. Die Unterscheidung in komplette und partielle Mole wird anhand morphologischer und histopathologischer Kriterien sowie aufgrund des Karyotyps getroffen. Die Therapie erfolgt mittels Kürettage oder Hysterektomie.
Wesentliches Element der Nachkontrolle sind engmaschige Bestimmungen des humanen Choriongonadotropins (HCG), das als klassischer Tumormarker dieser Erkrankungsgruppe dient. Bei Persistenz bzw. bei Vorliegen einer primär malignen Trophoblasterkrankung erfolgt die Therapie in Abhängigkeit vom Staging und von klinischen Prognosefaktoren, wobei für die Einteilung der Patientinnen in Risikogruppen mehrere Klassifikationssysteme zu Verfügung stehen.
Bei Patientinnen mit nichtmetastasierter Erkrankung oder metastasierter Erkrankung mit geringem prognostischem Risiko erfolgt die Monotherapie mit Methotrexat oder Dactinomycin. Bei Therapieversagen oder metastasierten Fällen mit hohem Risiko kann die Polychemotherapie in der Mehrheit der Fälle eine komplette Remission bewirken.
