Zusammenfassung

In diesem Kapitel werden die grundlegenden strukturellen Bestandteile und Funktionsmechanismen des Myometriums sowie äußere, vor allem hormonelle Einflüsse beschrieben, die auf die verschiedenen Gewebe der Gebärmutter wirken. Der Schwerpunkt liegt dann auf der Darstellung der Veränderungen, die sich im Laufe der Schwangerschaft ergeben und zu einer Zunahme der Kontraktilität bis hin zum Wehenbeginn führen.

Nach dem heutigen Stand des Wissens gibt es während der Schwangerschaft eine Vielzahl struktureller, humoraler und zellulärer Veränderungen, die letztendlich zur Auslösung regelmäßiger Wehen dienen. Ob dabei maternale, fetale oder plazentäre Faktoren den auslösenden Mechanismus darstellen, ist für den Menschen im Gegensatz zum Tier ungeklärt. Wichtig ist es, zu verstehen, daß es während der Schwangerschaft zu einem Zusammenspiel von schwangerschaftserhaltenden, das Myometrium relaxierenden sowie kontraktionsfördernden Faktoren kommt. Das zeitliche Zusammenspiel zwischen Wehenauslösung und Zervixreifung stellt eine Voraussetzung für eine komplikationsfreie Geburt am Termin dar.

In diesem Kapitel sollen die Faktoren zusammengefaßt werden, die für die zunehmende Kontraktilität im Veriauf der Schwangerschaft verantwortlich sind sowie für das Einsetzen der Wehentätigkeit am Termin.
