Zusammenfassung
Durch Entwicklung und Ausbau klinischer Untersuchungsmethoden wie Bronchoskopie, Thorakoskopie, Mediastinoskopie und perthorakale, CT-gesteuerte Lungenbiopsie können heute fast von jedem Lungentumor Gewebsproben für eine histologische oder zytologische Aufarbeitung gewonnen werden. Die diagnostische Bewertung durch den Pathologen ist dabei abhängig von Entnahmeort, Repräsentanz und Größe der Proben sowie Erhaltungs- und Fixierungszustand des Untersuchungsgutes. Klinische und statistische Untersuchungsergebnisse haben gezeigt, daß Krankheitsverlauf, Prognose und Therapie bösartiger Tumoren der Lunge entscheidend von Größe, Lokalisation und führendem histologischem Tumortyp zum Zeitpunkt der Diagnosestellung abhängig sind. Der Kliniker erwartet für seine therapeutischen Bemühungen vom Pathologen eine möglichst zweifelsfreie und klare Festlegung über die histologische und zytologische Charakterisierung des Tumors. Die morphologische Vielfalt der Tumoren — nur etwa 50% bösartiger Lungentumoren zeigen histologisch ein nahezu einheitliches Bild — hat in den letzten Jahrzehnten zu zahlreichen histologischen Klassifikationsvorschlägen geführt, die in die 1977 überarbeitete WHO-Klassifikation einmündeten (Tabelle 1) (Lit.-Übers.: Salzer 1967; Larsson u. Zettergren 1976; Yesner 1981; WHO 1982; Hirsch etal. 1983; Müller 1984, 1986; Mountain 1986).
