Zusammenfassung
Die Behandlung mit Valproinsäure ist bei einer großen Zahl von Epilepsiesyndromen im Kindesalter indiziert, wirksam und bei einigen für die andauernde Kontrolle epileptischer Anfälle zugunsten einer besseren Entwicklung unverzichtbar. Gerade in dieser Gruppe von Patienten, insbesondere bei den jüngeren Kindern, hat sich die schwerwiegendste Therapiekomplikation, das Leberausfallskoma als wesentlich häufiger gezeigt als bei älteren Kindern, Jugendlichen und Erwachsenen. Daher sind anhand der Erfahrungen in den Fällen von Lebertoxizität Parameter wie Ammoniak, Carnitin, Valproinsäuremetaboliten identifiziert worden, die im Rahmen von Belastungstests vor oder zu Beginn der Behandlung geprüft wurden. All diese Tests haben jedoch nicht einer wissenschaftlichen Prüfung standgehalten, und die dabei erhobenen Veränderungen stellen keine Kriterien von verläßlich prädiktivem Wert dar: Normale Befunde schließen die Möglichkeit einer Toxizität ebensowenig wie auffällige Befunde die Behandlung mit Valproinsäure begründet aus.
