Kardiologische Konsiliaruntersuchung und Behandlung bei Patienten mit allgemeinchirurgischen Eingriffen

Zusammenfassung

Untersuchungen aus den USA zeigen, daß von den 25 Mio, Patienten, die sich jährlich einer nichtkardialen Operation unterziehen, trotz der Erfolge in Diagnostik und Therapie, 50000 Patienten einen perioperativen Myokardinfarkt erleiden und mehr als die Hälfte der 40000 postoperativen Todesfälle durch ein kardiales Ereignis verursacht sind. Die häufigsten postoperativen Komplikationen bei Patienten über 40 Jahren sind kardiovaskulärer Natur (Eagle u. Boucher 1989; Goldman et al. 1977; Goldman 1988; Mangano 1990; Mangano et al. 1990), Allein die Kosten für den perioperativen Infarkt betragen 500 Mio Dollar/Jahr (Mangano et al. 1990).

Obwohl vergleichbare Daten aus der Bundesrepublik Deutschland nicht vorliegen (Statistisches Bundesamt, Wiesbaden 1992, persönliche Mitteilung) ist davon auszugehen, daß in der Bundesrepublik die Verhältnisse ähnlich liegen und auch bei uns die häufigsten postoperativen Komplikationen bei Patienten im Alter von über 40 Jahren kardiovaskulärer Natur sind. Dies hebt die Bedeutung der kardiologischen Konsiliaruntersuchung vor einem geplanten chirurgischen Eingriff zur Beurteilung der Operationsfähigkeit des Patienten und für die Festlegung der prä- und postoperativen Behandlung hervor.
