Perikarditis

Zusammenfassung

Eine Perikarditis bezeichnet isoliert oder sekundär auftretende Entzündungen des Herzbeutels, wobei die fibrinöse von der exsudativen Form abgegrenzt wird. Verschiedene Formen der akuten exsudativen Perikarditis neigen zur Persistenz; eine Perikarditis, die länger als 3 Monate verlauft, wird als chronisch bezeichnet. Während eine akut auftretende Flüssigkeitsansammlung im Herzbeutel, z.B. durch Blutung, schon bei 150–250 ml innerhalb von Minuten zur Herztamponade führen kann, werden chronische Ergüsse erst ab >250ml klinisch symptomatisch. Chronische Perikardergüsse, die bei ausreichender Dehnbarkeit des Perikards zu keiner Kompression führen, treten klinisch nicht oder nur diskret in Erscheinung. Wird die diastolische Füllung durch narbige Konstriktion des Herz, beuteis behindert, Hegt eine konstriktive Perikarditis vor. Die Diagnose eines Perikardergusses wird schnell und sicher durch die Echokardiographie gestellt.
