Endoskopische Therapie an Gallenwegen und Pankreas

Zusammenfassung

Seit der Einführung der endoskopischen Papillotomie im Jahre 1974 durch Classen und Demling [1] sowie Kawai [13] sind neben der Steinextraktion aus dem Gallengang mehrere therapeutische Eingriffe auf diesem Gebiet entwickelt worden. Einige von ihnen haben bereits die entsprechenden klassischen chirurgischen Verfahren ersetzt, andere stellen dagegen Alternativen bzw. Ergänzungen dar, die besonders bei alten und/oder schwerkranken Patienten anzuwenden sind:

Endoskopische Behandlungsmethoden an den Gallenwegen und am Pankreas

  • Papillotomie,

  • Steinextraktion aus dem Gahengang,

  • Lithotripsie (mechanisch, elektrohydrauhsch und Laser),

  • nasobiliäre Sonde zur Litholyse, endoluminalen Radiatio, ESWL und Spülung,

  • transpapihäre bilioduodenale Drainage,

  • suprapapilläre Fistelung,

  • Papillektomie bei Adenomen,

  • Bougierung bei Gallengangstrikturen,

  • Inzision des Pankreasgangorifiziums,

  • Steinextraktion aus dem Pankreasgang,

  • nasopankreatische Sonde (z. B. zur ESWL und zur Spülung bei infizierten Zysten),

  • transpapilläre transduktale Zystendrainage,

  • transgastrale oder transduodenale Zystendrainage,

  • transpapilläre Prothesenlegung in den Pankreasgang,

  • transpapilläre Fistelokklusion (z. B. mit Histoacryl oder Fibrinkleber),

  • Inzision der Papilla minor und Drainage beim Pancreas divisum.
