Zusammenfassung

In der BRD sterben jährlich über 80000 Menschen an einem Herzinfarkt. Bei mehr als der Hälfte der Patienten mit akutem Myokardinfarkt tritt der Tod in der 1. Stunde nach dem Infarkt ein, bedingt durch deletäre Herzrhythmusstörungen (meistens Kammernflimmern). Seit den 70er Jahren hat die Krankenhausmortalität des akuten Myokardinfarkts aus verschiedenen Gründen von 20–30% auf jetzt 10–15% abgenommen [1]. Von wesentlicher Bedeutung sind die Einrichtung von Intensivstationen, die Verbesserung der Überwachung und Reanimation in der Prähospital-Phase sowie neuere mechanische und medikamentöse Therapieverfahren. Diese Faktoren haben dazu beigetragen, daß Herzrhythmusstörungen als Todesursache im Verlauf des Myokardinfarktes ganz in den Hintergrund getreten sind, während die Linksherzinsuffizienz und der kardiogene Schock nunmehr eine herausragende Rolle spielen.
