Zusammenfassung
Die Mortalität des schweren akuten Lungenversagens des Erwachsenen (ARDS) beträgt auch heute noch 60–80% [4, 26]. Die hohe Mortalität beruht z.T. auf der für die schweren Formen erforderlichen aggressiven Beatmungstherapie selbst, die zur Progression des Lungenschadens beiträgt [10, 19]. Hierdurch kann sich ein verhängnisvoller Circulus vitiosus entwickeln, bei dem die teilweise iatrogen bedingte Zunahme der Lungenschädigung immer höhere Beatmungsdrücke und inspiratorische O2-Konzentrationen erfordert, Faktoren, die ihrerseits den Zustand der Lunge weiter verschlechtern, so daß schließlich das Krankheitsbild mit Hilfe konventioneller Beatmungsverfahren oftmals nicht mehr beherrscht werden kann. Wird bei diesen Patienten der extrakorporale Gasaustausch mit Membranlungen durchgeführt, so können die meist sehr hohen Beatmungsdrücke und Atemminutenvolumina weitgehend normalisiert werden. Läßt sich hiermit eine optimale extrakorporale CO2-Elimination erreichen, so kann die maschinelle Ventilation auf wenige Beatmungszüge reduziert und die Lunge im Sinne einer vorwiegend apnoeischen Oxygenation weitgehend ruhiggestellt werden. Hierdurch wird das Lungenparenchym vom schädigenden mechanischen Einfluß der extremen Beatmungstherapie entlastet; auch werden die ungünstigen Nebenwirkungen hoher intrathorakaler Drücke partiell aufgehoben [7, 8]. Weiterhin kann häufig auch der positive endexspiratorische Druck (PEEP) und (durch gleichzeitige Begrenzung des Inspirationsdrucks) der mittlere Atemwegsdruck vermindert werden. Allerdings ist es nicht immer möglich, auf hohe PEEP-Werte bzw. mittlere Atemwegsdrücke zu verzichten, da die Lungenexpansion und Reexpansion für die Aufrechterhaltung einer ausreichenden pulmonalen Oxygenation notwendig ist und wahrscheinlich einen wichtigen Faktor für die Gesundung der Lunge darstellt. Zumeist kann auch die inspiratorische O2-Konzentration (FIO2) auf ein nicht toxisches Niveau vermindert werden. Dies setzt bei schwerstem akutem Lungenversagen mit intrapulmonalen Rechtsmonalen Rechts-links-Shunts von >40% des Herzzeitvolumens (HZV) aber voraus, daß auch genügend Sauerstoff transferiert werden kann.
