Zusammenfassung
Nicht selten weisen chirurgische Patienten pathologische Abweichungen im Hormonhaushalt auf. In den meisten Fällen, wie z. B. bei Diabetes mellitus oder Hypothyreose, beruhen diese auf chronischen Funktionsstürungen inkretorischer Drüsen. Sie künnen allerdings auch iatrogen durch längerdauernde Hormontherapie verursacht worden sein, wie z.B. bei langfristiger Glukokortikoidtherapie. Gelegentlich sind hormonproduzierende Tumoren, wie z. B. kleinzellige Bronchialkarzinome, Hypophysen- oder Schilddrüsenadenome oder ein Phäochromozytom, sowohl Ursache der endokrinen Stürung als auch die eigentliche Operationsindikation. Unbehandelte endokrine Erkrankungen sind in der perioperativen Phase eine potentielle Gefahr für den Patienten. Die Patienten benütigen eine besondere präoperative Evaluation und ein entsprechendes perioperatives Management. Wie schon bei gesunden Patienten beruht sowohl die fundierte Risikoabschätzung als auch die Entscheidung für eine bestimmte Anästhesieform und die Wahl der erforderlichen Monitoringmethoden auf einer sorgfältigen Anamnese und auf einer das Wesentliche berücksichtigenden kürperlichen Untersuchung.
