Synopsis

  1. 1.

    Repolarisationsveränderungen im EKG während einer Lithiumbehandlung sind dosisabhängig, reversibel und ohne klinische Bedeutung.

  2. 2.

    Arrhythmien sind bei therapeutischen Lithiumkonzentrationen im Serum selten.

    Es handelt sich überwiegend um Sinusknotendysfunktionen und Reizleitungsverzögerungen im Bereich der Vorhöfe und des AV-Knotens (supra-His) mit sekundärem Auftreten von Ersatzrhythmen bzw. Extrasystolen.

  3. 3.

    Pathophysiologisch scheint diesen Störungen eine Zunahme der Dauer des Aktionspotentials mit Verlängerung der funktionellen Refraktärzeit sowie eine Abnahme der Depolarisationsgeschwindigkeit (Phase 4 — Depolarisation) zugrunde zu hegen.

  4. 4.

    Relative Kontraindikationen für die Behandlung mit Lithium sind dementsprechend Erkrankungen mit gehäuften Sinusknotendysfunktionen und Bradyar-rhythmien wegen der Gefahr synkopaler Anfälle.
