Zusammenfassung
Die Linse hat optische Aufgaben zu erfüllen — daneben ist von untergeordneter Bedeutung, daß sie den Augen-Innenraum untergliedert. Das Linsengewebe nimmt nicht nur innerhalb des Auges, sondern auch im Gesamtorganismus eine Sonderstellung ein: Die Linse ist ein rein epitheliales Gebilde und von der 3. embryonalen Lebenswoche an durch eine Basalmembran vom übrigen Organismus im Sinne des Wortes abgekapselt (Smelser, 1965; Wulle u. Lerche, 1967). Es fehlt eine direkte Versorgung mit Gefäßen, Nerven und auch der unmittelbare Kontakt mit mesenchymalen Zellen (s.Kap. 1). Diese im Körper einzigartige Isolation geht einher mit einer Reihe von Besonderheiten, die für die Pathologie der Linse von entscheidender Bedeutung sind (Abb. 9.1, 2; s. Kap. 1):

Selbstverständlich kommen primäre Entzündungen nicht vor, ebenso wenig wie neurogene Veränderungen.

Obwohl das Linsenepithel physiologisch bis ins hohe Lebensalter proliferiert, sind primäre Neoplasien unbekannt. Letzteres läßt sich evtl. ebenfalls durch das Fehlen der Gefäßversorgung erklären, insbesondere wenn man den von Folkman et al. (1971) postulierten Angiogenesefaktor als Voraussetzung für ein progressives neoplastisches Wachstum akzeptiert. Ida Mann (1947) hat aber gezeigt, daß Linsenepithel, vermischt mit kanzerogener Substanz und subkutan injiziert, befähigt ist, ein anaplastisches Linsenkarzinom zu induzieren.

Durch den Druck der zeitlebens anhaltenden Proliferation des Linsenepithels und der ständig nachrückenden Linsenfasern findet bereits physiologischerweise eine Degeneration der langgestreckten Zellen im Linsenkern statt: Sie zeigt sich an der Kernsklerose des alten Menschen, die ohne scharfe Grenze in eine Kern-Katarakt übergeht.

Bemerkenswert ist das organ-spezifische Antigen des Linseneiweißes der Wirbeltiere. Der Körper behandelt das eigene Linsengewebe als Fremdprotein, sobald es nicht mehr von der Kapsel umschlossen ist und bildet Antikörper dagegen. Linsen verschiedener Wirbeltiere haben gewisse Antigeneigenschaften gemeinsam, und zwar nimmt ihre Zahl stufenweise zu, je näher die Tierarten miteinander verwandt sind. Nach Halbert und Manski (1963, 1965) lassen sich durch das Studium der feinen Unterschiede im Linsenantigenverhalten der verschiedenen Vertebraten sogar Einblicke in die Evolution im Tierreich gewinnen. Die organspezifischen Antikörper sind scheinbar nicht befähigt, Linsenzellen zu schädigen (Halbert u. Manski, 1963) — von engverwandten Spezies. Eine zytotoxische Wirkung in der Linsenzellkultur entfalten nur sehr heterologe Antilinsenseren.

Jede Linsenkapseleröffnung beim Menschen induziert eine phakogene Uveitis (Zimmerman, 1964). Die klinisch und histopathologisch eigentümliche, sogenannte Endophthalmitis phakoanaphylactica (Von Domarus et al., 1975; Verhoeff u. Lemoine, 1922; Yanoff u. Scheie, 1968) oder akute phakolytische Glaukome (Völcker u. Naumann, 1975) stellen Prozesse dar, die wahrscheinlich auf zellständige. Antikörper zurückzuführen sind (Marak, 1971; Rahi et al., 1977, s.Kap. 4).

Die gefäßlose, vom Kammerwasser umspülte Linse ist in gewisser Weise mit einer Gewebs- oder Organkultur vergleichbar. Es gelingt tatsächlich auch relativ leicht, Linsenzellen in einer Gewebekultur lebensfähig zu halten (Vogel und Reinoehl, 1957) — sogar über Jahre hinweg (Van Den Veen und Heyen, 1959). Diese Möglichkeit hat Untersuchungen des Stoffwechsels der Linse — sowie ihres elektrischen Potentials stimuliert (Constant, 1957, 1959; v. Heyningen, 1976; Hockwin und Cole, 1974; Kinoshita, 1974; Müller et al., 1950, 1956; Nordmann, 1972; Pau et al., 1973; Pirie, 1962). Offenbar vermögen bestimmte Substanzen (z.B. Kortikosteroide, Zytostatika, Dimethylsulfoxyd) aus dem Kammerwasser die Linsenkapsel leicht zu durchdringen und eine abnorme Proliferation oder degenerative Veränderungen zu erzeugen (s. experimentelle Katarakte).
