Zusammenfassung

Zur Strahlenbehandlung von Koronarstenosen stehen unterschiedliche Systeme zur Verfügung, gas- oder flüssigkeitsgefüllte Ballonkatheter, Stents oder Strahlendrähte und Strahlenzüge, die nach dem Afterloading-Prinzip passager in ein Koronargefäß eingebracht werden (Tabelle 8.1). Das Zielvolumen für eine Bestrahlung ist klein. Typischerweise werden Segmente von 2–5 cm Länge, mit einem Durchmesser von 3–5 mm und einer Gefäßwanddicke von 0,1–1,5 mm behandelt. Für die katheterbasierte Strahlentherapie werden im Wesentlichen 3 Isotope verwendet (Tabelle 8.2), 192Ir (γ-Strahler mit 612 keV maximaler Energie, Halbwertszeit 74 Tage), 90Sr/90Y (β-Strahler mit 2270 keV maximaler Energie, Halbwertszeit 28 Jahre) und 32P (β-Strahler mit 1710 keV maximaler Energie, kurze Halbwertszeit von nur 14 Tagen) [9].
