Rethoracotomy in Thoracic Surgery

Summary

A total of 1516 thoracic surgical procedures were performed from January 1986 to September 1990. In 14 patients (0.9%), 17 rethoracotomies (1.1%) were necessary: three patients each underwent two procedures. The most frequent complication was bleeding (10/17), followed by sepsis (5/14). Three septic comphcations were due to bronchial stump leakage. One rethoracotomy was necessary for chylothorax and one for oncologic reasons. Two patients who suffered recurrent bleeding died. For the typical lung resection (n = 531), bleeding was also the most frequent comphcation (4/7 rethoracotomies). The three remaining rethoracotomies were for empyema due to bronchial stump leakage. The total rethoracotomy rate was 1.3%.

Zusammenfassung

Von Januar 1986 bis September 1990 wurden bei 1516 thoraxchirurgischen Eingriffen bei insgesamt 14 Patienten (0,9%) 17 Rethorakotomien (1,1%) durchgeführt, davon jeweils 2 bei 3 Patienten. Häufigste Komplikation war die Blutung (10 von 17 Eingriffen), gefolgt von septischen Komplikationen (5/14) 3mal als Folge einer Bronchusstumpffistel. Je eine Rethorakotomie war wegen Chylothorax und aus onkologischen Gründen erforderhch. Zwei Patienten mit rezidivierenden Blutungskomplikationen sind verstorben.

Bei den typischen Lungenresektionen (n = 531) stand die Blutung ebenfalls an erster Stelle (4 von 7 Reeingriffen insgesamt), die übrigen 3 Rethorakotomien waren wegen Empyem bei Bronchusstumpffistel erforderlich. Gesamtrethorakotomierate 1,3%.
