Computed Tomography of the Abdomen in postoperative ICU Emergencies

Summary

From 1985 to 1990,171 CT scans of abdomen and thorax (n = 35) from 114 surgical ICU patients were evaluated and compared to intraoperative state and chnical outcome. Indications for CT were: search for focus of sepsis (n = 125), primary disease (n = 106), bleeding (n = 26), trauma in = 21) checking course of disease (n = 62) and postoperative complications (n = 92). In 88 CTs intervention was recommended: in 40 cases surgery followed immediately; in 10 cases CT-guided drainage was successful and in two cases surgery, followed unsuccessful CT drainage. In the 83 CTs in which no intervention was recommended, surgery ensured in only six cases. Hence 45% of all CTs made surgery unnecessary.

Zusammenfassung

Von 1985 bis 1990 wurden bei 114 chirurgischen Intensivpatienten 171 Abdomen- und Thorax-CT’s mit dem OP-Befund und klin. Verlauf verghchen. CT-Indikationen waren (Mehrfachn. möglich): Suche nach Sepsisherd 125x, Grundkrankheit 106x, Blutung26x, Traumen21 x, Verlaufskontrollen 62x, post-op. Komphkationen 92x. 88x wurde durch die CT eine Intervention nahegelegt. 40x wurde sofort operiert. 10x wurde erfolgreich und 2x erfolglos (mit anschl. OP) CT-gesteuert drainiert. Bei 83 CT’s, die keine Intervention nahelegten, erfolgte nur 6x eine OP. Letztendlich dienten 45% aller angefertigten CT’s der Absicherung gegen ein operatives Vorgehen.
