Prehospital Treatment of Torso Injuries

Summary

Respiratory insufficiency requires early artificial respiration. A pneumothorax should be drained, at least, in the patient being respirated, after digital opening of the thorax by silicon drainage. In mediastinal emphysema with serious haemodynamic consequences collar mediastinotomy is indicated. If there is a pericardiac tamponade with circulatory collapse after a perforating trauma, the patient must be seen by a surgeon as quickly as possible. If patient with a rupture of the trachea cannot be respinated, immediate exposure of the rupture site is imperative.

Zusammenfassung

Die respiratorische Insuffizienz erfordert die frühzeitige Beatmung. Ein Pneumothorax sollte zumindest bei einem beatmeten Patienten drainiert werden. Die Entlastung sollte nach digitaler Eröffnung des Thorax durch eine Silicon-Drainage erfolgen. Liegt ein Mediastinalemphysem mit erheblicher hämadynamischer Auswirkung vor, ist die Entlastung durch kollare Mediastinotomie angezeigt. Liegt eine Pericardtamponade mit drohendem Kreislaufversagen nach perforierendem Thoraxtrauma vor, muß der Patient schnellstmöglich einer chirurgischen Behandlung zugeführt werden. Kann ein Patient mit Trachealruptur nicht beatmet werden, ist die sofortige Freilegung der Rupturstelle dringhch.
