Life Threatening Bleeding from Pelvic Vessels in Multiple Trauma Patients — The Value of Angiographic Embolisation

Summary

Since 1984 18 multiple trauma patients with pelvic fractures and arterial bleeding from pelvic vessels received angiographic diagnosis and consecutive embolization by transcatheter angiography. Seven Patients survived. The overall time delay until bleeding localization was 7 h, requiring a median substitution of 9 units of blood and 3 FFP’s. Three patients died immediately, three others died of severe head injuries. Five patients died of septic multiorgan failures. This last group had a threefold increased time delay until embolization, requiring an increase of substitution by factor 3 (med. 21 h, 30 units of blood, 14 FFP’s). Quoad vitam an early diagnosis of arterial bleeding from pelvic vessels is of highest importance and in cases of persisting bleeding early angiography is mandatory.

Zusammenfassung

Seit 1984 erfolgte bei 18 polytraumatisierten Patienten mit Beckenfrakturen die angiographische Lokahsation und Embolisation massiv blutender arterieller Beckengefäße (vergleichbare Verletzungsschwere Grad III-IV, Hannover PTS). Das zeithche Intervall bis zur angiographischen Blutungslokalisation und Embolisation der pelvinen Blutungen betrug bei sieben Überlebenden median 7 Stunden mit einem medianen Substitutionsbedarf von 9 EK’s und 3 FFP’s. Drei schwerverletzte Patienten verstarben unmittelbar, drei erlagen den Folgen eines SHT. Fünf Patienten verstarben im septischen Multiorganversagen. In dieser Gruppe war ein 3fach längeres Intervall bis zur Blutungslokalisation sowie ein dreifach höherer Substitutionsbedarf auffällig (med. 21 Std., 30 EK’s, 14 FPP’s). Quoad vitam ist zur frühen Diagnose bei Blutungspersistenz die Angiographie essentiell.
