Surgical Ultrasound — 8 Years of Experience in Patients with Multiple Injuries

Summary

Between 1981 and 1989 clinical examination of 366 patients with multiple injuries was performed by ultrasound (56.% intubated). In blunt abdominal trauma lavage was abolished and replaced by ultrasound with a reliability of 100% in patients with intraabdominal bleeding needing immediate laparotomy. Surgical ultrasound examination includes transcutanous chest (hemothorax) and heart (pericardial effusion) ultrasound under emergency conditions. Transoesophageal endosonography after severe thoracic trauma in intubated patients is the method of choice in suspected aortic lesions. These primary investigations are supplemented with ultrasound examination if no life-threatening injuries are found. Arthrosonography of the shoulder (effusion, rotator cuff tear) and knee joint (cruciate ligament lesion, meniscus tear, effusion), achilles tendon and blunt muscular trauma may lead to further diagnostic measurements or operative treatment. Our clinical experience proves that ultrasound performed by the experienced surgeon on duty is highly reliable, gives excellent information and allows immediate action for the benefit of the patient.

Zusammenfassung

In den Jahren 1981 — 1989 wurden 366 Mehrfachverletzte neben der üblichen Diagnostik sonographisch beurteilt. (58,8% intubiert) Beim stumpfen Bauchtrauma hat die Sonographie mit einer Treffsicherheit von fast 100% in der Hand des erfahrenen Chirurgen eine Lavage ersetzt. Während freie Flüssigkeit ab 50 ml sicher zu erkennen ist, bleibt die Organläsion in ihrer genauen Lokahsation schwierig. Richtungsweisend ist die Flüssigkeitsansammlung in der betroffenen Region. Die Sofortdiagnostik schließt Herz (transcutanes Echo: Pericardeinblutung) und Thorax (Hämatothorax) neben dem Abdomen mit ein. Bei schwerem Thoraxtrauma mit Verdacht auf eine Aortenläsion kann die transoesophageale Sonographie richtungsweisend werden. Gelenke (Schulter: Erguß, Rotatorenruptur) (Knie: Bänder, Meniscen) und Weichteil Verletzungen (Muskel, Sehnen, Hämatome) werden in der Sekundärphase untersucht; Hämatome unter sonographischer Kontrolle entlastet. Die klinischen Erfahrungen bestätigen die publizierte Effizienz der nebenwirkungsfeien, sicheren und schnellen diagnostischen Methode Ultraschall und bekräftigen die Forderung nach chirurgische Sonographie — Diagnose und Operation bleiben in einer Hand.
