Die schwere Leberruptur im Rahmen des Polytrauma — eine Herausforderung an den Chirurgen

Summary

Some 29 of 62 polytraumatized patients with rupture of the liver (July 1986-December 1990) showed major parenchymal destruction which was unilobar in 15 patients (Grade IV Moore) or bilobar in 14 patients (Grade V). Grade V rupture was in all cases associated with injury of the hepatic veins and in 8 patients with lesions of the retrohepatic vena cava. Surgical treatment involved resection (n = 18), suture (n = 4), suture of the cava (n = 5), packing (n = 8; in addition n = 5); no therapy was possible in 4 patients. The mortahty rate was 62% (n = 18), with hemorrhage as the most common cause of death (n = 10) due primarily to the rupture of the vena cava.

These patients benefit only from direct surgical intervention, otherwise conservative organ preserving therapy was pursued.

Zusammenfassung

Bei 62 Polytraumen mit Leberruptur (1. 7. 86-31. 12. 90) fanden sich 15mal einseitige Parenchymzerstörungen (Grad IV n. Moore) und 14mal beidseitige (Grad V) stets in Kombination mit Lebervenen-, 8mal mit retrohepatischer Cavaverletzung. Therapie: Resektion n = 18, Parenchymnaht n = 4, Cavanaht n = 5, Tamponade n = 8 (additiv n = 5), keine Therapie möglich n = 4.19 Patienten befanden sich im hämorrhagischen Schock, 6mal erfolgte die Laparotomie unter Reanimation. Blutungsbedingte Todesursachen lagen bei 10 der 18 Verstorbenen vor. Ursächlich war in erster Linie die Cavaruptur. Hier ist bei ansonsten organerhaltendem Therapieziel nur nach sofortigem Erkennen und direkter Nahtversorgung ein Überleben möglich.
