Summary
The long-term clinical and functional results of coloanal anastomosis (CAA) in the management of low and midrectal cancer were analyzed and compared with an age-matched group of patients with abdominoperinal resection (APR). No patient died as a result of pelvic sepsis. Anastomotic leakage occurred in 7% (handsewn 20%, stapled 3%), anastomotic strictures in 2.4%. Nine months after CAA, complete or nearly complete continence was achieved by 85% of the patients. More than 3 years postoperatively (1977–1987), 57 patients with curative resection could be analyzed. After a mean (± SD) length of follow-up of 6.8 years (range 3–13.6 years), local recurrence cumulative rates were 11% after CAA and 17% after APR, distant recurrence rates were 33% and 34%, respectively. The cumulative 5-year survival rate was 62% for patients with CAA and 53% for patients with APR. From these results we conclude that intersphinteric resection with CAA is a safe and efficient alternative to APR in many distal rectal carcinomas.
Zusammenfassung
Die klinischen und funktionellen Ergebnisse der intersphincteren Rectumresektion mit direkter coloanaler Anastomose (CAA) (n = 85) bei der Behandlung von Carcinomen im mittleren und unteren Rectumdrittel werden mit einer entsprechend strukturierten Gruppe von Patienten mit Rectumexstirpation (APR) (n = 27) verglichen. 62mal befand sich der Tumor im unteren, 23mal im mittleren Rectumdrittel. Kein Patient verstarb an den direkten Folgen einer pelvinen Sepsis. Klinisch relevante Anastomoseninsuffizienzen traten in 7% auf (Handnaht 20%, Klammernaht 3%), Anastomosenstrikturen in 2,4%. 9 Monate nach CAA waren 85% der Patienten komplett oder nahezu komplett kontinent. 57 von 63 Patienten, die zwischen 1977 und 1987 operiert wurden, konnten kurativ reseziert werden. Nach einem mittleren Nachbeobachtungszeitraum von 6,7 Jahren (3–13,6 Jahre) betrug die kumulative lokale Rezidivrate nach CAA 11%, nach APR 17%. Fernmetastasen wurden in 33 resp. 34% beobachtet. Die 5-Jahres-Überlebensrate betrug nach CAA 62%, nach APR 53%. Diese Ergebnisse bestätigen, daß die CAA in vielen Fällen eine sichere und effiziente Alternative zur APR darstellt.
