Summary
Examinations of 635 biopsies and 163 surgical specimens resulted in a morphological description of characteristics of small cell cancer of the lung (cell diameter 9 µm, medial diameter of nuclei 7 µm). Histological diagnosis reflects only a rough phenotypic parameter of this tumor entity. At thorough examination of small cell cancer squamous, adenoid and/or giant cell differentiation can be found within the same tumor tissue in about 20% of specimes. Histological examination of spontaneous and therapeutical induced tumor regression in resected specimens yielded findings such as necrosis, scar development and endangitic blood vessel transformations. Results of electron microscopy, immunohistochemistry and in situ hybridisation also underline a wide range of heterogeneity in the special group of small cell cancer of the lung. The documentation of a broad spectrum of morphological findings in specimens from resected tumors demonstrates that a 2-mm biopsy represents only a rough parameter of a biologically complex tumor.
Zusammenfassung
635 Bronchusbiopsien und 163 Operationspräparate mit Lungentumoren wurden unter der Frage einer eigenen Tumorentität kleinzelliger Karzinome der Lunge analysiert. Die entscheidende Primärdiagnose basiert auf den phänotypischen Befunden relativ kleiner Tumorzellen (Zelldurchmesser ca. 9 µm, Kerndurchmesser ca. 7 µm). In 20% sind in vorwiegend kleinzelhgen Lungentumoren auch adenoide, plattenepithehale und/oder großzellig differenzierte Anteile vorhanden. Elektronenoptisch und immunhistochemisch ist eine qualitativ und quantitativ unterschiedhche Sekretionsleistung bevorzugt von Peptidhormonen zu demonstrieren. Zur Bestimmung der Tumorregression in Operationspräparaten nach vorausgegangener Chemotherapie kleinzelliger Karzinome werden drei Regressionsgrade von fehlender bis zu kompletter Regression mit Nekrosen, Makrophagenaktivierungen, Vernarbungen und endangitischen Gefäßveränderungen vorgeschlagen. Die morphologischen Untersuchungsergebnisse belegen eine große Tumorheterogenität als Erklärung der biologischen Variabilität auch in der Gruppe kleinzelliger Lungentumoren. Dennoch ist die Führung als besondere Tumorentität auch morphologisch gerechtfertigt
