Die Bedeutung der Leukozytenelastase in der Frühdiagnostik postoperativer Komplikationen nach allgemeinchirurgischen Eingriffen

Summary

In a prospective clinical trial the value of leucocyte elastase in early diagnosis of postoperative inflammatory comphcations was analysed. Some 103 patients after small and great abdominal and thoracal operations were studied. Among 15 of 17 postoperative comphcations we saw an early increase of elastase in comparison to other inflammatory parameters and before clinical manifestation of comphcation. Leucocyte elastase seems to be another sensitive parameter in diagnosis of septic and nonseptic complications.

Zusammenfassung

In einer prospektiven klinischen Studie wurde die Wertigkeit der Leukozytenelastase in der Frühdiagnostik postoperativer entzündhcher Komphkationen nach 103 kleineren und großen abdominellen und thorakalen Eingriffen analysiert. Bei 15 von 17 postoperativ aufgetretenen Komphkationen sahen wir einen frühzeitigen Anstieg der Elastase im Vergleich zu den übrigen Entzündungsparametern und vor klinischer Manifestation der Komphkation. Die Leukozytenelastase erwies sich als weiterer sensibler Entzündungsparameter in der Differentialdiagnose septischer und nicht septischer Komphkationen. Ein normales postoperatives Elastaseverhalten schloß eine septische Komphkation aus.
