Radical Systematic Lymphadenectomy in Bronchial Carcinoma — Preliminary Results of a Controlled Prospective Randomized Clinical Trial

Summary

The value of radical systematic lymphadenectomy (LA) in the treatment of bronchial carcinoma is controversial. In a controlled trial, we compared LA with conventional lymphadenectomy (LS). Comparison of short-term results revealed a significantly longer operation time, and higher blood loss in the LA group. Mortality at 30 days was comparable in both groups. The incidence of postoperative complications was significantly increased in LA. In conclusion, LA leads to a better staging of bronchial carcinoma, but is associated with significantly higher morbidity.

Zusammenfassung

Der Wert der radikalen systematischen Lymphadenektomie in der Therapie des Bronchialkarzinoms ist umstritten. Im Rahmen einer prospektiven kontrolherten randomisierten Studie wurde daher die radikale systematische Lymphadenektomie (LA) mit der konventionellen Lymphadenektomie mit Lymphknoten-Sampling (LS) verghchen. Insgesamt gingen bisher 114 Pat. in die Studie ein, wobei auf die Gruppe LS 56 und auf die Gruppe LA 58 Pat. entfielen. Bei den intraoperativen Parametern zeigte sich eine signifikante Erhöhung der OP-Dauer, des Blutverlustes und des Bluttransfusionsbedarfes in der Gruppe LA. Die 30 Tage-Letahtät war in beiden Gruppen vergleichbar, insgesamt war die postoperative Komphkationsrate in der Gruppe LA signifikant höher. Zusammenfassend führt die radikale systematische Lymphadenektomie zwar zu einem verbesserten Staging des Bronchialkarzinoms, jedoch auf Kosten einer signifikant erhöhten Morbidität.
