Zusammenfassung
Postoperative Infektionen nach gynäkologisch-geburtshilflichen Eingriffen sind Komplikationen, die Arzt und Patientin belasten. Sie beeinträchtigen stark das Allgemeinbefinden der Patientin und können unter Umständen lebensbedrohliche Folgen haben. Liegedauer und Krankenhauskosten erhöhen sich. Die Verhütung von Infektionen durch Verminderung oder Beseitigung ihrer Ursachen setzt die Kenntnis der pathogenetischen Faktoren voraus. Exogene Krankenhausinfektionen, die durch Hände (90%), Gegenstände, Instrumente,Luft und Flüssigkeiten übertragen werden, sind leichter zu bekämpfen als endogene, die einen Anteil von 30–50% aller Infektionen haben. Mikroorganismen der körpereigenen Flora (endogene Infektion), wie z. B. von Magen-Darm-Trakt, NasenRachen-Raum und Haut, können unter bestimmten Umständen die Ursachen einer infektiösen Erkrankung des Individuums werden. Dies trifft für die infektiösen Erkrankungen in der Geburtshilfe und Gynäkologie in besonderem Maße zu. Schon in der Scheide gesunder Frauen und symptomfreier Schwangerer können im wesentlichen alle Keime beobachtet werden, die ggf. eine Infektion verursachen (Tabelle 1). Die unterschiedliche Häufigkeit, mit der bestimmte Keime von einzelnen Autoren beobachtet werden, kann durch den sozioökonomischen Hintergrund, durch das Sexualverhalten und durch klimatische Verhältnisse bedingt sein. Fehler bei der Entnahme des Untersuchungsmaterials, beim Transport und bei der mikrobiologischen Bearbeitung beeinflussen Zahlenangaben ebenso wie Verschiedenartigkeit und unterschiedliche Anzüchtbarkeit der Keime. In den letzten Jahren hat sich unser Wissen um die aerobe und anaerobe bakterielle Flora des Genitaltraktes durch gezielte Untersuchungen bestimmter Mikroorganismen erweitert [18, 26, 38, 78, 87, 105, 106, 114].
