Zusammenfassung

Das Lymphogranuloma inguinale stellt nicht — wie die an sich etwas unglücklich gewählte, aber einmal eingebürgerte Bezeichnung zu sagen scheint — einen auf die Leistenlymphknoten beschränkten Krankheitsprozeß dar. Es handelt sich vielmehr um eine ausgesprochene Allgemeinkrankheit, die in ihrem Verlauf ohne weiteres mit der Lues verglichen werden kann. Auch hier ist als Initialerscheinung ein meist genital lokalisierter Primäraffekt vorhanden, der jedoch — da nicht selten unauffällig, uncharakteristisch und flüchtig — meist unbemerkt bleibt, so daß gewissermaßen als Leitsymptom zuerst die vergrößerten, später in charakteristischer Weise einschmelzenden und fistelnden regionalen Lymphknoten wahrgenommen werden, die über viele Wochen das Bild beherrschen können. Daneben wird das Allgemeinbefinden durch Fieber, Kopfschmerzen, Abgeschlagenheit und Appetitlosigkeit und einen damit verbundenen Gewichtsverlust mehr oder minder stark beeinträchtigt. Diesen Früherscheinungen können manchmal schon nach Wochen, in der Regel jedoch erst nach Ablauf einiger Jahre, gelegentlich noch nach Jahrzehnten Spätmanifestationen folgen, die als außerordentlich chronische, fast stets progrediente Entzündungsprozesse in Form der genitoanalen Elephantiasis mit oder ohne Ulcerationen sowie der entzündlichen Rectum-striktur zu den gefürchtetsten Erscheinungsformen des Lymphogranuloma inguinale gehören. Diese auch therapeutisch oft nur unzureichend zu beherrschenden Veränderungen führen im Laufe der Zeit nicht selten zu schweren Zerstörungen ihrer Umgebung; die Beckenorgane können in den Krankheitsprozeß mit einbezogen werden, ausgedehnte Abscesse und Fistelsysteme bilden sich aus; viele Rectumstrikturen machen Colostomien nötig, die nicht selten durch Coecooder Ileostomien ersetzt werden müssen, weil die Entzündung nun auch weiter proximal gelegene Darmabschnitte ergriffen hat. Sekundärinfektionen, Perforationen oder die fortschreitende Kachexie führen in vielen Fällen ein frühzeitiges letales Ende herbei, andere Kranke erwartet ein jahrzehntelanges Siechtum.
