Zusammenfassung

Der Isthmus der Aorta ist die Strecke des auslaufenden Aortenbogens, die zwischen der linken Schlüsselbeinader und dem Abgang des Ductus Botalli liegt. Dort können gewisse Einengungen auch häuüg bei der Obduktion Erwachsener nachgewiesen werden, obwohl nie eine klinisch bemerkbare vorgelegen hat. Dieses Stück des Aortenrohrs bildet sich in der zweiten Hte des Embryonebens. Die verschiedene Länge des Isthmus hängt von der sog. Wanderung des Ursprungs der A. subclavia bzw. vom Deszensus des Herzens ab. Von einer Isthmusstenose oder Koarktation spricht man dann, wenn die Enge durch Erhöhung des Strömungswiderstandes hämodynamisch so gravierend wird, daß sich neben der suprastenotischen Hypertension eine mangelnde, im wesenchen über die Kollateralgefäße erfolgeride Minderdurchblutung der unteren Körperhte mit den entsprechenden Symptomen einstellt. Die Krankheitszeichen beginnen bei typischen Isthmusstenosen selten im Kleinkindesalter, sondern meist erst im 14.—15. Lebensjalir, wenn man an die überwiegenden postduktalen Stenosen denkt, die das Hauptkontingent der Operierten darstellen.
