Zusammenfassung

Die radikale Entfernung der Bauchspeicheldrüse beim Menschen ist in mehrfacher Hinsicht problematisch: Die versteckte Lage der Drüse und ihre nahe topographische Beziehung zu anderen Organen und vor allem zu den großen Gefäßen bedingt operativ-technische Schwierigkeiten besonderer Art. Die Verhältnisse der regionalen Gefäßversorgung zwingen in der Regel zur Mitnahme weiterer Organe wie des Duodenums, der Milz und eines Teiles des Magens, was den Eingriff zu einem der schwersten in der gesamten Chirurgie macht. Der Ausfall der inkretorischen und exkretorischen Funktion der Bauchspeicheldrüse bedarf einer dauernden und sehr sorgfältigen Substitutionstherapie. Obwohl die rein technischen Fragen der Operation schon vor über 50 Jahren gelöst erschienen, ermöglichten erst die neueren Fortschritte der Schock- und Infektionsbekämpfung, der Anaesthesie und vor allem der Stoffwechselforschung eine häufigere Vornahme derartiger Eingriffe.
