Die Wegenersche Granulomatose

Zusammenfassung

Wegener hat 1936 und 1939 als „rhinogene Granulomatose“ ein Krankheitsbild beschrieben und abgegrenzt, das sich im Anfangsstadium klinisch durch entzündliche Veränderungen im Bereich von Nase, Nebenhöhlen oder Mittelohren auszeichnete. Der weitere Verlauf war charakterisiert durch unklare septische Erscheinungen, als terminale Symptome stellten sich schwere Funktionsstörungen der Nieren ein. Als pathologisch-anatomische Hauptbefunde wurden eine nekrotisierende Entzündung im Gebiet der oberen Luftwege, eine der Periarteriitis nodosa weitgehend ähnliche generalisierte Arteriitis, multiple Granulombildungen in den inneren Organen (besonders den Lungen) sowie eine herdförmige granulomatöse Glomerulonephritis nachgewiesen. In pathogenetischer Hinsicht vermutete Wegener eine hyperergische Reaktion des Organismus auf einen primären, in Nase und Nebenhöhlen zu lokalisierenden infektiösen Prozeß.
