Abstract
Aortico-pulmonary septal defect, or aortico-pulmonary fistula, is an uncommon congenital anomaly. In symptomatology and pathophysiology it is almost identical with patent ductus arteriosus of comparable size and form. Embryologically and topographically, on the other hand, the two lesions are dissimilar. Differentiation between them by clinical methods is scarcely possible. As a septal defect the aortico-pulmonary anomaly may be compared to ventricular septal defect, the embryologic development of wrhich is also similar. Clinically and functionally, aortico-pulmonary septal defect further resembles a communication between the aorta and the pulmonary artery arising from aneurysmal rupture. SCHATTENBERG and HARRIS (1943) reported 62 cases of the latter type. The congenital communication is not tubal, but is always of window type. Its size varies. In the 14 collected cases of SPENCER and DWORKEN (1950) the diameter of the defect ranged from 5 to 60 mm. ; in 6 cases it was 10 mm. The defect is always centrally situated, close to the semilunar valves.
