Zusammenfassung
Die Behandlung der Lungentuberkulose hat sich im Laufe der letzten zehn Jahre grundlegend geändert. Dank der Chemotherapie kann heute eine große Zahl von Tuberkulosekranken geheilt werden. Jede aktive Tuberkulose erfordert den Einsatz dieser Medikamente. Es muß heute geradezu als Kunstfehler bezeichnet werden, wenn er unterbleibt. Es gelingt damit, vor allem die frischen tuberkulösen Herde zur Rückbildung zu bringen und die Tuberkelbacillen aus dem Auswurf zu eliminieren. Die tuberkulostatische Behandlung soll dabei, um Resistenzbildungen zu vermeiden, kombiniert vorgenommen werden (E. Bernard, Kreis, Chrétien und Jullien; King; Tanner u. Mitarb.). Die gleichzeitige Kombination aller drei sog. klassischen Tuberkulostatica (Streptomycin, PAS, INH) bringt aber gegenüber der Kombination von zweien keine wesentlichen Vorteile (E. Bernard, Kreis, Chrétien und Jullien; King; Wier, Storey, Tempel und Weiser). In Amerika und in Holland wird diese medikamentöse Behandlung sehr lange fortgeführt, nämlich im allgemeinen 6–12 und mehr Monate lang. Ross und Kay sahen überhaupt keine Rückfälle, wenn sie nach Verschwinden der ursprünglichen Kaverne noch während mindestens 6 Monaten fortgesetzt wurde, während die Zahl der Rezidive beträchtlich war, wenn die tuberkulostatische Therapie nach Kavernenschluß weniger als 4 Monate lang weitergeführt wurde. Wier, Storey, Tempel und Weiser fanden bei 354 tuberkulösen Kranken nach 6 monatiger Behandlung mit verschiedenen Kombinationen von Tuberkulostatica in 95–98% negative und nur noch in 2–5% positive Kulturen.
