Postthorakotomiesyndrom und Wundheilungsstörung — Probleme in der Rehabilitation aus der Sicht des Chirurgen

Zusammenfassung

Das Postthorakotomiesyndrom wurde erstmals nach Mitralkommissurotomie beschrieben [17, 18]. Als Ursache wurde eine Reaktivierung eines rheumatischen Fiebers vermutet. Da das typische klinische Bild eines Postthorakotomiesydroms auch nach anderen herzchirurgischen Eingriffen mit Eröffnung des Perikards auftrat, wurde die „Reizung des Perikards“ traumatisch oder durch Fremdkörper verursacht, als auslösende Ursache dieses Syndroms angesehen [8, 12, 13]. Als Dressler [7] 1959 die gleiche Symptomatik im Anschluss an einen Myokardinfarkt beschrieb, wurde als Ursache des klinischen Bildes sowohl nach herzchirurgischen Eingriffen als auch nach Myokardinfarkt ein durch traumatisiertes oder zugrunde gegangenes Myokardgewebe entstandenes Autoantigen angesehen. Die Isolierung von Autoantikörpern gegen Herzgewebe unterstützte diese Theorie [1, 9, 15], wobei jedoch keine Korrelation vom klinischen Beschwerdebild und dem Nachweis von Antikörpern beobachtet wurde.
