Zusammenfassung
Die linksventrikuläre Hypertrophie stellt einen wichtigen eigenständigen Risikofaktor für subsequente kardiovaskuläre Ereignisse dar. Neben hämodynamischen Ursachen, insbesondere der arteriellen Hypertonie, lassen sich in epidemiologischen Studien wie den MONICA-Surveys weitere Faktoren identifizieren, die die linksventrikuläre Muskelmasse beeinflussen. Hierzu gehören das Geschlecht, die Körpermasse, das Körperfett, der Hämatokrit, das Alter, neurohumorale und genetische Faktoren. Die Methode der Indexierung der linksventrikulären Masse spielt für die interindividuelle Vergleichbarkeit und damit für die Diagnose einer linksventrikulären Hypertrophie eine entscheidende Rolle. So ist insbesondere beim Vorliegen einer Adipositas eine Indexierung nach der fettfreien Masse vorgeschlagen worden. Für den Hypertrophieprozess scheint die basale Aktivität des Renin-Angiotensin-Systems eine bedeutende Rolle zu spielen. Zahlreiche Studien konnten hierbei einen modulierenden Einfluss des Insertions-/Deletionspolymorphismus im ACE-Gen in Kombination mit oder auch ohne hämodynamische Belastung des linken Ventrikeld nachweisen.
