Zusammenfassung
Kardiovaskuläre Erkrankungen, insbesondere die koronare Herzkrankheit (KHK), sind die führenden Ursachen für Morbidität und Mortalität bei Frauen über 50 Jahren, während kardiovaskuläre Erkrankungen bei jüngeren Frauen selten sind. Diese Tatsache, die insgesamt geringere Inzidenz und das um 10–15 Jahre verspätete Auftreten der KHK bei Frauen, hat zu der kontrovers diskutierten Frage geführt, ob die Menopause ein Risikofaktor der KHK ist.
Es besteht eine allgemeine Übereinstimmung, dass die chirurgisch-induzierte vorzeitige Menopause nach bilateraler Ovarektomie ein Risikofaktor der KHK ist, der mit einem relativen Risiko von 2 verbunden ist, sofern Östrogen nicht ersetzt wird. Studien zur natürlich auftretenden Menopause zeigten variable Ergebnisse. Dieser Review fokusiert auf die Kriterien, die für die Anerkennung einer Assoziation als Risikofaktor gefordert werden. Das Auftreten der natürlichen Menopause wird durch bestehende Risikofaktoren in unterschiedlicher, teilweise entgegengesetzter Weise beeinflusst. Rauchen führt zu einem vorzeitigen Auftreten der Menopause, Übergewicht, Hypertonie und Diabetes zu einem späteren Auftreten. Diese komplexen Interaktionen komplizieren die statistische Analyse. Die natürliche Menopause ist mit eingreifenden biochemischen und metabolischen Veränderungen verbunden, die ihrerseits etablierte Risikofaktoren sind, z.B. ein Anstieg des LDL-Cholesterins und eine Abnahme des HDL-Cholesterins. Diese biochemischen und metabolischen Veränderungen sind die Mechanismen, über die die Menopause als Risikofaktor wirken kann.
Daher kann die Menopause als Risikofaktor akzeptiert werden, selbst wenn nicht alle Voraussetzungen, insbesondere die Reversibilität, erfüllt sind. Das Auftreten der Menopause zeigt den Übergang von einem niedrigen zu einem hohen Risiko der KHK bei Frauen an.
