Zusammenfassung

Qualitätsmanagement ist in der Medizin von zunehmender Bedeutung (18). Was Qualität ist, ist nur im Hinblick auf explizite Standards beurteilbar. Standards können prinzipiell anhand statistisch-quantitativer und/oder qualitativer Normen definiert werden — in der Regel müssen sich beide ergänzen. Dabei müssen — soweit vorhanden — die Ergebnisse empirischer Therapie- und Versorgungsevaluationen berücksichtigt werden. Im Sinne einer „evidence-based medicine“ (13, 29) bzw. „evidence-based psychiatry“ (20) muß die medizinische Praxis auf den bestmöglichen Informationen basieren, die aus den Ergebnissen wissenschaftlicher Standards genügender Studien gewonnen werden. Eine evidenzbasierte Medizin legt weniger Gewicht auf Intuition und (unsystematische) klinische Erfahrung als auf rationale Grundlagen für klinische Entscheidungen und betont dabei die Suche nach empirischen Belegen durch die klinische Forschung (Evidence-Based Medicine Working Group 1992, zit. nach 20). Dazu muß der Arzt mit wissenschaftlichen Denkmethoden vertraut und in der Lage sein, sich über das vorhandene empirische Wissen zu informieren. In modernen Informationssystemen verfügbare Studienergebnisse, Metaanalysen oder Leitlinien bieten zunehmend die Grundlage hierfür (z.B. 6,7).
