Zusammenfassung
In einer randomisierten, doppelblinden, Placebo-kontrollierten Multicenter-Studie untersuchten wir die Wirkung von Candesartan cilexetil (Cand.cil.), einem neuen Angiotensin-II-Typ-1-Rezeptorblocker mit langanhaltender antihypertensiver Aktivität, auf den Glukosestoffwechsel — und das Serumlipidproiil — bei Patienten mit milder Hypertonie und stabilem Typ-II-Diabetes mellitus.
Insgesamt wurden 161 Männer und Frauen im Alter von 30–75 Jahren mit milder Hypertonie (diastolischer Blutdruck im Sitzen gemessen 90–100 mmHg) und Typ-II-Diabetes mellitus (Hämoglobin A1c 5,5–9,0%), beides nach einer 4-wöchigen Placeborun-in-Phase gemessen, für eine doppelblinde Therapie mit Cand.cil. 8 mg einmal täglich (n = 83) oder Placebo (n = 78) randomisiert. Die Dosis wurde auf 16 mg einmal täglich erhöht, falls der diastolische Blutdruck ≥ 90 mmHg blieb.
Bei Randomisierung und nach 12-wöchiger Therapie wurden HbA1c (primäre Zielvariable), Blutzucker und Parameter des Serumlipidproiils (u.a. Gesamtcholesterin, HDL- und LDL-Cholesterin, Triglyceride) bestimmt.
Die statistische Auswertung der Unterschiede zwischen den beiden Therapieformen beruhte auf Änderungen der Parameter vom Randomisierungszeitpunkt bis zum Ende der Studie.
Cand.cil. hatte keinen signifikanten Einfluß auf HbA1c, Blutzucker und Serumlipide im Vergleich zu Placebo. Das mediane HbA1c war bei Studienbeginn und nach 12 Wochen 7,1% bei Patienten unter Cand. Cil., und 7,2% bzw. 7,1% bei Padenten unter Placebo.
Das 95%-Konfidenzintervall für den medianen Unterschied zwischen den Gruppen war eng (-0,25; 0,16), und umfaßte Null, was jeden klinisch bedeutsamen Unterschied ausschloß. Dies galt auch für den Blutzucker (-1,10; 0,20), das Gesamtcholesterin (-0,40; 0,20) und die anderen Lipidparameter. Mehr als 60% der Padenten erreichten einen diastolischen Blutdruck < 90 mmHg, Nebenwirkungen und Studienabbrüche waren in beiden Gruppen gleich.
Zusammenfassend beeinflußt Candesartan cilexedl in einer Dosis von 8–16 mg einmal täglich bei Padenten mit milder Hypertonie und Typ-II-Diabetes mellitus weder den Glukosestoffwechsel noch die Serumlipide. Der Blutdruck wurde bei den meisten Patienten kontrolliert und Candesartan cilexetil wurde gut vertragen.
