Zusammenfassung
Calciumantagonisten sind eine chemisch sehr heterogene Substanzklasse mit Phenylalkylamin-, Dihydropyridin- und Benzothiazepinstruktur. Trotz ihrer chemisch unterschiedlichen Strukturen besitzen sie Gemeinsamkeiten in ihrem pharmakokinetischen Verhalten. Obwohl die Substanzen nach oraler Gabe nahezu vollständig resorbiert werden, ist ihre biologische Verfügbarkeit aufgrund eines ausgedehnten präsystemischen Metabolismus oft gering. Der Grund für ihr ähnliches Verhalten in Pharmakokinetik und Metabolismus dürfte darin liegen, daß diese Arzneimittel durch die gleichen Cytochrom P 450-Enzyme metabolisiert werden. Ein weiterer wichtiger Aspekt der klinischen Pharmakologie der Calciumantagonisten ist die Stereochemie dieser Arzneimittel. Mit Ausnahme von Nifedipin, das ein achirales Dihydropyridinderivat ist, besitzen alle therapeutisch eingesetzten Calciumantagonisten mindestens ein Chiralitätszentrum und sind somit optisch aktiv. Es sind zwei sterische Konfigurationen möglich, die sich wie Bild und Spiegelbild verhalten und die Ebene des polarisierten Lichtes entweder nach links oder rechts drehen. Bis auf Diltiazem, das als cis-(+)- Stereoisomer vorliegt, sind alle zur Zeit therapeutisch eingesetzten chiralen Calciumantagonisten Razemate, das heißt sie sind eine 50:50-Mischung von links- und rechtsdrehendem Enantiomer. Sowohl im Hinblick auf ihre Wirksamkeit als auch ihre Pharmakokinetik unterscheiden sich die Enantiomere teilweise erheblich. Bei Gabe des Razemates wird somit nicht ein Arzneimittel, sondern die fixe 50:50-Kombination zweier Calciumantagonisten verabreicht. In diesem Kapitel werden Daten zur Resorption, Proteinbindung, zum Metabolismus und zur Pharmakokinetik therapeutisch häufig eingesetzter Calciumantagonisten besprochen, wobei der Schwerpunkt auf Gallopamil liegt.
