Zusammenfassung

Calciumantagonisten vermindern in vitro die Freisetzung entzündlicher Mediatoren in der Lunge und hemmen den Ca2+ -Einstrom in glatte Muskelzellen der Atemwegsmuskulatur durch spannungsabhängige Ca2+-Kanäle. Die Lungenfunktion in Ruhe bleibt durch Calciumantagonisten unbeeinflußt. Am hyperreaktiven Bronchialsystem können sie eine Bronchokonstriktion, die durch körperliche Belastung, durch cholinerge Agonisten und Histamin sowie durch Allergenexposition induziert wird, kurzfristig unterdrücken. Nifedipin und aerosolisiertes Gallopamil sind die effektivsten bronchodilatatorischen Calciumantagonisten. Kontrollierte Studien zur Behandlung manifester obstruktiver Atemwegserkrankungen mit Calciumantagonisten stehen aus. Bei der Therapie kardiovaskulärer Krankheiten bei Patienten mit Atemwegserkrankungen sind β-Rezeptorenblocker kontraindiziert. Hier stellen Calciumantagonisten eine wichtige Alternative dar.
