Zusammenfassung
Einleitung: Als Alternative zur Photoablation mit dem Excimerlaser (193 nm, UV) gilt der Erbium-YAG-Laser (2,9 μm, Infrarot). Zur Myopiekorrektur ist (in erster Näherung) entsprechend einer umgekehrten parabolischen Kurve im Zentrum der Ablationszone eine tiefere Hornhautabtragung notwendig als im peripheren Bereich. Beim Erbium-YAG-Laser kann daher im Fundamentalmode die Erzeugung des Ablationsprofils vereinfacht werden.
Material und Methoden: Bei dem verwendeten Laser handelt es sich um einen Erbium-YAG-Laser der im Fundamentalmode als Energieprofil (Fluence, in Joule pro cm2 in Abhängigkeit vom Ort) eine Gauß-Verteilung zeigen sollte. Um das Energieprofil zu bestimmen, wurden die Fluencewerte an verschiedenen Profilpunkten mit dem Joulemeter gemessen. Darüber hinaus wurden Hornhäute (Schwein) mit einer durchschnittlicher Fluence zwischen 0,8–2,9 J/cm2 bestrahlt, um das Ablationsprofil mit den gemessenen Werten zu vergleichen. Die Ablationsherde wurden photografisch dokumentiert, die Hornhäute wurden präpariert, in Formalin fixiert und mit Hämatoxilin und Eosin gefärbt. Die Schnitte durch das Ablationszentrum wurden photo-graphiert, um das Ablationsprofil zu gewinnen.
Ergebnisse: Messungen mit dem Joulemeter zeigen, daß das Energieprofil des Laserstrahls einer Gauß-Verteilung entspricht. Photoablation findet in einer Zone von 3,5 mm Durchmesser statt. Das Ablationsprofil der Hornhäute entspricht einer umgekehrten parabolischen Kurve mit weitgehend homogenem Rand und geringen thermischen Reaktionen.
Schluβfolgerungen: Der Erbium-YAG-Laser im Fundamentalmode ist eine aussichtsreiche Alternative zur herkömmlichen Photoablation mit dem Excimerlaser, die neben rascher Ablation eine technische Vereinfachung durch Wegfall der herkömmlichen Blende ermöglicht.
Summary
Objective: Photovaporisation with the erbium:YAG laser (λ = 2.94 μm) is caused by energy absorption in water molecules. For myopia correction, a deeper ablation in the center than in the periphery is required. Such a profile can be approximated by a gaussian-curved fluence as produced by an Er:YAG laser running in fundamental mode, offering simplification of the instrument.
Materials and Methods: An Er:YAG laser running in fundamental mode was used to evaluate the energy profile. Measurements obtained by the joulemeter were compared to the ablation profiles of cadaver pig eyes.The pig eyes were treated with different fluences (0.8–2.9 J/cm2 on average) and, after histological, preparation morphologically examined using light microscopy and SEM.
Results: Measurements with the joulemeter and in the cadaver pig eyes showed a gaussian-curved energy profile (ablation). In this setting, the diameter of ablation was 3.5 mm. Histological examination showed a homogeneous profile of ablation with minor thermal damage.
Conclusion: The Er:YAG laser, running in its fundamental mode, allows homogeneous ablation of corneal tissue. In addition to the easier technical handling of the Er:YAG laser (compared to the excimer laser), it has none of the potential risks of UV light.
