Zusammenfassung
Hintergrund: Die erste Hinterkammerlinse, die vor 45 Jahren von Ridley implantiert wurde, war eine Disklinse. Die Idee nach erfolgter Kataraktoperation eine den Kapselsack zirkulär ausspannende Kunstlinse zu implantieren, wurde seitdem mehrmals aufgegriffen, fand aber nicht auf breiter Ebene Zuspruch. Über das Langzeitfixationsverhalten solcher Linsen in menschlichen Augen ist wenig bekannt.
Material und Methoden: Fünfzehn Autopsieaugen, die mit einer Linse mit modifizierten, zirkulären Haptikdesign nach Anis implantiert waren, wurden untersucht. Die Augen wurden äquatoriell eröffnet und der Kapselsack mit Implantat von glaskörperwärts photodokumentiert. Fixation, Zentrierung, Kapselsackmorphologie, Beschaffenheit der Zonulae und die Entwicklung der zentralen Cataracta secundaria und eines Soemmerring-Ringes wurden analysiert und mit den Ergebnissen aus Autopsieaugen mit PMMA-Hinterkammerlinsen (HKL) verglichen.
Ergebnisse: Alle 15 Anis-Linsen waren symmetrisch im Kapselsack fixiert. Die mittlere Dezen-trierung der Anis-Linsen war signifikant geringer als die der Kontrollgruppe (p = 0,02, Varianzanalyse). Die Augen mit der Anis-Linse zeigten weniger Zonuladeformierungen (p = 0,0005) und eine bessere zirkuläre Ausspannung des Kapselsackes (p = 0,003, Kruskal-Wallis-Test). Zwischen der Anis Linse- und Standard-HKL-Designs konnte kein statistisch signifikanter Unterschied in bezug auf die Nachstarrate (p = 0,29) und Soemmerring-Ringbildung (p = 0,67, Kruskal-Wallis-Test) nachgewiesen werden.
Schlußfolgerungen: Das Konzept einer Linse mit zirkulären Haptiken bzw. einer Disklinse bot Vorteile in bezug auf das Kapselsackzentrierverhalten. Ein Unterschied in der peripheren oder zentralen Nachstarbildung ließ sich nicht nachweisen.
Summary
Background: The first posterior chamber intraocular lenses (IOLs) implanted by Harold Ridley 45 years ago had a circular disc-like design. Since then, several disc IOLs have been developed but have never gained real wide-spread usage. Therefore, little is known about the long-term performance of this lens type.
Material and Method: Fifteen autopsy eyes that had been implanted with an Anis circular IOL were evaluated. The eyes were sectioned at the equatorial plane and the anterior segment with the capsular bag and the IOL were photographed from the posterior perspective. Fixation, centration, capsular bag shape, zonula damage, and development of posterior capsule opacification (PCO) and Soemmerring’s ring (SR) formation were analyzed. The results were compared with a control group consisting of autopsy eyes with one-piece and three-piece PMMA IOL designs.
Results: All 15 Anis IOLs showed symmetrical in-the-bag fixation. The mean decentration of the Anis IOL was significantly lower than that of the control group (p = 0.02, analysis of variance). The eyes with the Anis lens showed fewer zonular defects (p = 0.0005) and a better circular extension of the capsular bag (p = 0.003, Kruskal-Wallis test). There was no statistically significant difference between the Anis and control group in terms of PCO (p = 0.29) and SR formation (p = 0.67, Kruskal-Wallis test).
Conclusions: Thus, an IOL with a circular haptic design showed advantages in terms of centra-tion and capsular bag morphology. A difference in terms of PCO or SR development could not be shown in the relatively small sample size.
