„Pigmentoasa-IOL“: Möglichkeit zur Optimierung des Raumempfindens bei peripherem Gesichtsfeldverlust

Zusammenfassung

Auf der Basis intensiver Beschäftigung mit dem— von uns optimierten —„teledioptrischen System“ nach Koziol/Peyman zur operativen Rehabilitation bei Makuladegeneration entwickelten wir ein konträres System „Pigmentosa-IOL“, das bei peripherem Gesichtsfeldverlust („Rührenblick“) und monokularem Eingriff am schlechterem Auge Raumempfinden und damit Flexibilität im Alltag wiederherstellen kann.

Das System besteht in einer Doppelimplantation: einer speziellen kapselsackfixierten Hinterkammerlinse mit vorerst standardisiertem optischen Anteil von + 60 dpt, kombiniert mit einer VKL von—25 dpt. Anhand einer 65jährigen Patientin mit der bislang längsten Follow-up-Zeit (9 Monate) sollen Methode, mögliche optisch-physiologische Hintergründe, subjektive Befindlichkeit und zukünftige Anwendungsmöglichkeiten diskutiert werden.

Summary

A new version of capsular bag IOL (+ 60.0 D) in combination with a standby IOL (-25D) is able to provide a practical sense of visual room to persons with peripheral visual field deficiencies, e.g. retinitis pigmentosa patients. The result is that orientation in a familiar environment is possible. After a simulation test the worst of the two eyes with only small remnants of visual field should be operated on. In this way, a pseudovisual field of 45°—or even more—can be restored.

We present the experiences of a 65-year-old woman after a follow-up time of 9 months. Objective results: the ocular hydrodynamics were normal and unchanged on both sides. The anatomical situation was constant. Subjective experience: She can move without stretching out her arms to find exits, doors or objects by feeling. She is able to go out of home without help and she watches TV without interruptions, deviations or disturbances.
