Zusammenfassung
Problemstellung: Bei der Behandlung der kindlichen Katarakt mit intraokularen Linsen ist die Wahl der Refraktion problematisch, da im Laufe des Lebens aufgrund des Bulbuswachstums eine Änderung der benötigten IOL-Brechkraft auftritt. Zur Lösung dieses Problems haben wir eine Sandwich-IOL vorgeschlagen, die auf einer PMMA-Trägerlinse eine entfernbare Zusatzlinse aufweist. Untersucht werden soll die Frage, ob mit einem derartigen System eine befriedigende, aniseikoniearme Korrektur zu erzielen ist.
Methode: Die theoretischen Werte für die Aniseikonie bei einseitiger IOL-korrigierter Aphakie mit Brillenkorrektur des Refraktionsdefizites wurden rechnergestötzt ermittelt. Zugrundegelegt wurden hierbei die aus der Literatur bekannten Werte für die altersabhängige Brechkraft der menschlichen Linse für das gesunde Auge. Aus der Differenz zur angenommenen Brechkraft des Implantates kann für verschiedenste Kombinationen die Aniseikonie errechnet werden.
Ergebnisse: Die Simulation einer Sandwich-IOL von 20 dpt Brechkraft mit einer Zusatzrefraktion von 8 dpt, die im zweiten Lebensjahr entfernt wird, zeigt eine maximale Aniseikonie von unter 5%. Eine konventionelle IOL mit 20 dpt Brechkraft erzeugt eine fast dreimal so große Aniseikonie. Die Sandwich-IOL ermöglicht eine deutlich bessere Annäherung an die angestrebte Iseikonie.
Summary
Problem: The treatment of infantile cataracts by implantation of intraocular lenses is difficult when it comes to the choice of the refractive power, as in the course of a lifetime the required IOL power changes due to bulbar growth. To address this problem, we suggested a sandwich-IOL, which has a removable adnex lens on top of a PMMA carrier lens. In this study, we determined if such a system is suited to achieving satisfactory correction including minimal aniseikonia.
Methods: The theoretical values of aniseikonia in unilateral IOL-corrected aphakia with correction (glasses) of the refraction deficit were calculated by a computer-aided method and based on the values of the age-dependent refractive power of the human lens in phakic eyes, determined from the literature. From difference between these values and that of the refractive power of the implant, aniseikonia can be determined for different combinations.
Results:Simulation of a sandwich-IOL of 20 dpt refractive power and an additional refraction of 8 dpt, removed at the age of 2 years, showed maximal aniseikonia ranging below 5%, whereas a threefold aniseikonia is generated by a conventional IOL of 20 dpt refractive power. The sandwich-IOL enables much better approximation of the desired iseikonia.
